Are you a film user? We have provided the form below that you can print out at home, fill out and include it with your film. Be sure to print out the postage-paid mailing label, attach the label to your envelope and simply drop it in the mail.

Place your film in a secure padded envelope along with the order form. Please remember to indicate print size and other options you wish to receive. Affix the postage-paid label onto the envelope and place in a U.S. mailbox. For your convenience, we will always include an additional personalized mailer with your completed order.
